Think of the TCH as a virtual urgent care center for St. Paul’s PACE. With just one phone call, you can quickly access all of your PACE services, from urgent clinical consultations to routine care.
It’s like having PACE on speed dial, making healthcare more accessible and easier to coordinate.
On the Horizon
Our cutting-edge software will soon enable on-site nurses to remotely monitor your health and proactively respond to changes.
They’ll track these changes in real-time and provide immediate assessment when necessary—whether through telehealth, home health services, or an urgent in-person visit at one of our PACE sites.
Care Made Simple
Our new PACE Concierge service is also coming soon, designed to make care coordination even easier for our participants. This service will provide live, on-demand updates on transportation, caregiver availability, and upcoming appointments with PACE providers and other specialists.
